To ensure we can perform the most comprehensive and precise research for you, please follow these 4 steps.

Before proceeding, you must fill out and sign the digital confidentiality waiver.
Step 2: Functional Status Assessment Questionnaire
Please complete this short questionnaire to assess the current functional status.

Collecting Medical Documents

Medical Summary
- Summary of recent visit to the oncologist
- Current referral letter
- List of regular medications for background diseases (if any)
Important: Include background on co-morbidities (if any) besides cancer.

Histological Diagnosis (Crucial)
- Pathology reports from ALL biopsies performed * You may attach up to 5 documents if available
- Immunohistochemistry (IHC) staining
Please attach all historical reports, not just the most recent one.

- Interpretation reports of recent imaging (CT, PET-CT, MRI) * You may attach up to 5 documents if available
- Ultrasound / X-ray

- Liver/Kidney function and CBC

- Protocols of past chemotherapy/radiation
